Registration

To register you will need to live within the practice area.

Either visit the surgery to register, or print out and fill in the Registration form GMS1, (link below) and post it to Number 18 Surgery, 18 Upper Oldfield Park, Bath, BA2 3JZ. This form will need to be filled in fully and then signed by one of the partners at the surgery, before you will be registered with us.

Reception will supply you with a brochure and timetable and give you a brief description of the surgery, or this can be viewed on the surgery website.

If you are from overseas and have not been registered in the UK before, you will need to present a valid work permit/proof of employment.

Students from overseas will need a student visa and confirmation of the university they are attending.

Sick Notes

The rules on sick notes are:

For the first three days of an illness your employer does not legally require any sick notes, unless stated in your contract.

For days 4, 5, 6 and 7 you can give your employers a ‘Self Certificate’ sick note (click here to view and print a copy).  This is a form you complete and does not require a doctor's opinion.

For any illness over 7 days long you will need a doctor’s note.

If your employer wants a doctor’s note for a period of illness less than a week we are not obliged to issue any certificate, if we do there may be a fee involved for your employer, see below. We will happily discuss this with your employer with your permission.

In special circumstances we can issue a private sick note for a charge of £10.
